HospiceMed, LLC, and its affiliates (collectively, “HospiceMed”), recognizes the importance of protecting the privacy of information we may receive from visitors to our website(s). You may visit our website(s) without telling us anything about yourself, and our website(s) do not currently employ any technology that would collect any domain or other information from your browser or computer. If you contact us and choose to provide personally identifiable information, we will utilize it to respond to your inquiry. We do not collect or maintain personally identifiable information for resale purposes or share your information with any other company or organization. Any and all uses comply with applicable laws.

In some cases, HospiceMed’s use of information that is provided by the user will also be subject to requirements of the Health Insurance Portability and Accountability Act (“HIPAA”). This will be the case when you submit information to HospiceMed for transmission of prescription and patient information data for subsequent transmission to and dispensing from a designated pharmacy of the user’s choice.

Registered Access

The majority of the HospiceMed related websites are accessible only to registered users. To become a registered user, you must belong to a business entity that has contracted with HospiceMed to provide prescribing or other related pharmacy software services. Your employer or HospiceMed will provide you with the information you need in order to complete the registration process.

Personally Identifiable Information

HospiceMed collects personally identifiable information of website users, such as names, business address, e-mail addresses, phone number and the like, only when voluntarily provided by a website visitor. HospiceMed additionally collects protected health information as voluntarily submitted by the website user in the process of transmitting prescription data information. This may include patient name, patient address, patient date of birth, last four of the patient social security number, patient diagnosis, prescription drug information and patient allergy information. The use of this information will be further subject to the terms of the End User Licensing Agreement in place between HospiceMed and the website user when entering and submitting information.

Unsecured Email Warning

Please note that any information you send to HospiceMed via email is sent via an unsecured email link. Due to the nature of the Internet, there is a possibility that unsecured (unencrypted) email could be intercepted and read by third parties. HospiceMed assumes no responsibility for interception of confidential information you send in an unsecured (unencrypted) email message.
